Composition and function of Bile -Bile salts -Phospholipids -Protein -Cholesterol -Bilirubin -Usual electrolytes of plasma. It plays an important role in fat digestion and absorption: Emulsification of large fat particles into minute particles that can be digested by the lipase enzyme secreted in pancreatic juice. Aid in the transport and absorption of digested fat end products to and through the intestinal mucosal membrane. It helps in the excretion of several important waste, these include bilirubin ( end product of hemoglobin destruction) and the excess cholesterol synthesized by the liver cells.

Kőképződés A bilirubin pigmentkövek keletkezésében az epe vegyi összetételének változása játszik szerepet. Pigmentkövek gyakrabban képződnek a közös epevezetékben. Kockázati tényezői a krónikus hemolízis (vérsejtoldódás), az alkoholos cirrózis (májzsugorodás), epefertőzések, magas életkor.
Kőképződés Leegyszerűsítve azt mondhatnánk a koleszterinkövek képződéséről, hogy magképződés jön létre a túltelített epében, melyet a koleszterinelválasztás növekedése hoz létre. Ez tehát egy anyagcserezavar, melynek során ún. litogén (kőképző) epe, majd epekő keletkezik.
Kőképződés Létrejöttüket elősegítő tényezők még az epehólyag működési zavarai, az ezt követő pangás, esetleges gyulladás, baktériumok (E. coli), a fokozott mucinképződés (ez egy bizonyos szénhidrátot és fehérjét tartalmazó nyálkahártyaváladék). A koleszterinkövek rizikófaktorainak tanulmányozása során öt jellegzetes tulajdonságot szokás kiemelni: a páciens különösen hajlamos rá, ha szőke, kövér, nő, (többször volt) terhes, és negyvenes éveiben jár.

Gallstones Classification and Nomenclature Fajtájuk szerint az epekövek lehetnek koleszterinkövek (gyakoriságukat az irodalom 60-90% között adja meg), bilirubin vagy pigmentkövek (6-15%-ban fordulnak elő), továbbá kevert kövek (3440%). Következmények, szöv ődmények Az epekövek az esetek többségében némák maradnak (vagyis a betegek lényegében panaszmentes epekőhordozók), esetleg kisebbnagyobb emésztési zavarokból álló panaszokkal jelentkeznek. Ha az epekő "megszólal", akkor súlyos, igen intenzív, görcsös fájdalommal jelentkezik (epekőkólika).
Következmények, szöv ődmények Gyulladások: epehólyaggyulladás elzáródással, folyadékgyülem, gennyesedés esetén gennygyülem; következményes hasnyálmirigygyulladás; epehólyagfalelhalás, kilyukadással, vérzéssel, más üreges szervbe való átjutással és ezek súlyos következményeivel.
Következmények, szöv ődmények Epehólyagkövek vándorlása járhat beékelődéssel, sárgasággal, bélbe jutva bélelzáródással. Hosszú idejű fennállása esetén idült májmegbetegedéshez, májzsugorhoz vezet, rosszindulatúan elfajulhat.

Kőoldás Bizonyos epesavak szerepének tisztázásával megtalálták a kőoldáshoz szükséges gyógyszert, melyek nemcsak az epe koleszterintelítettségét csökkentik, hanem ezáltal epekőképződési hajlamát is. A koleszterinkövek epesavakkal oldhatóvá váltak. Ajánlható az oldó kezelés akkor, ha az epehólyag jól "működik", és koleszterintartalmú epekövesség áll fenn.
Kőoldás Gombostűfejnyi (1-2 mmes) kő oldása 2-3 hónapot, cseresznyényi (1215 mm-es) kőé 12-24 hónapot igényel, de az orvosi szakirodalomban beszámoltak már 3 cm nagyságú kő eredményes oldásáról is. Természetesen az oldó terápia sikere függ nemcsak a kövek nagyságától, hanem számától is. Az epeköves betegeknek csak 20-25%a alkalmas oldó terápiára. Sikeres kezelésük érdekében megváltoztatjuk a betegek étkezési szokásait: rostdús, kalóriaés koleszterinszegény étrendet tanácsolunk, napi többszöri étkezéssel. Tréningeztetjük az epehólyagot, de nem provokáljuk. Ultrahangvizsgálattal követhetjük nyomon az oldó kezelés eredményességét. A siker az irodalmi adatok szerint széles skálán mozog: 30-70%. Öt év elteltével a betegek 45-50%-ában kiújulással lehet számolni. A kezelés ismételhető, és kombinálható, pl. külső lökéshullámú kőzúzással.
Kőoldás Ismeretes az úgynevezett direkt kőoldás is, amelynek keretében két lehetőség kínálkozik az oldószer bejuttatására az epehólyagkövek direkt oldása céljából: bőrön és májon keresztüli behatolással vagy szájon keresztül, ún. endoszkópos úton. Elterjedését a szövődmények gyakorisága, a kezelés bonyolultsága gátolta.
Kőzúzás A technika fejlődésével, mint az oldó kezeléseknél, megjelentek az ún. kőzúzó berendezések. A zúzásra használt energia eljuttatása a kövekhez szintén kétféle úton lehetséges: direkt (testen belüli), indirekt (testen kívüli) úton.
Kőzúzás Nem elterjedt beavatkozás a direkt úton történő kőzúzás. Főleg közös epevezetékkövek eltávolítására használható. Az energiát közvetlen úton juttatjuk a kövekre. Bőrön és májon keresztül, speciális eszköz segítségével jutunk az epehólyagba és távolítjuk el, törjük szét az epeköveket a PTCL-kezelés során.
Kőzúzás A kezelés előfeltétele: jól működő, folyadékot tartalmazó epehólyag. Hátránya, hogy pénzigényes az endoszkóp, a zúzóberendezés. kombinálható az oldó kezeléssel
Kőzúzás 1979-ben kezdődtek az angol rövidítéssel ESWL-nek nevezett indirekt, "testen kívüli" kőzúzás kísérletei, 1980-ban kezdték alkalmazni a vese, veseútköves megbetegedések kezelésére. Lényege, hogy a fókuszált külső lökéshullámok azonos hangsűrűségű anyagokon (víz, lágyrész) energiaveszteség nélkül áthaladnak, a más hangsűrűségű anyagokban (kő) azonban energiájukat leadják, kioltódnak.
Kőzúzás Ultrahang vagy röntgen segítségével történik a célzás, mm pontossággal. A külső lökéshullám által zúzott 1-3 mm-es morzsalékok az epeutakon keresztül a bélbe jutnak. Bebizonyosodott, hogy az ESWL-kezelést indirekt oldó kezeléssel kombinálva, a kőmentesség ideje jól kimutathatóan csökkent.
Kőzúzás Javallható az ESWL-kezelés a következő esetekben: panaszokat okozó epehólyagkövesség; jól működő epehólyag; maximum 3-5 darab, 5 mm-től 3 cm-ig terjedő nagyságú, egyedülálló, meszet nem tartalmazó koleszterinkő; együttműködés a beteg részéről a kezelés előtt, alatt és a követésben.
Kőzúzás Fontos előnyökkel rendelkezik ez a kezelési mód: megtartja az epehólyagot; biztonságos, haláleset nem ismert; a lökéshullámok komputer által vezérelve, a szívütések között érik el a köveket. ismételhető, ambulanter elvégezhető. Kombinálható direkt, indirekt kőoldással, PTCLkezeléssel. Az ESWLkezelést monoterápiaként nem alkalmazzuk. Mindig kiegészítjük indirekt kőoldással.
Kőzúzás Nem szabad elhallgatnunk viszont azt sem, hogy az epehólyagköves betegek csak 15-20%ban kezelhetők ESWL- és vele kombinált oldó terápiával. A berendezés drága, a követés időigényes. ESWLkezelésnél is fontos a betegek korrekt kiválasztása, lelkiismeretes kezelése, rendszeres követése és az étkezési szokásaik megváltoztatása.

Cont Management Conservative: NPO, NGT, iv fluids, Ab Operative Cholecystectomy (optimum) If cholecystectomy cannot be done, cholecystostomy may be appropriate. Gallbladder content is drained. This can be done percutaneously under US guidance or operatively. Cholecystectomy may be undertaken at a later date.

A long standing obstruction may lead to biliary cirrhosis & portal HTN Presentation H/O biliary colic Jaundice, Pale bulky stool & tea colored urine May have pruritis, nausea & vomiting O/E: jaundice
Pancreatitis Due to the presence of stones in duodenal papilla. Symptoms (pain in RUQ & epigastric, fever). Investigation ( leukocytosis, high amylase & lipase, U/S shows stones in pancreatic duct). Treatment supportive ERCP to remove stones from pancreatic duct and biliary tree through papillatomy. early cholecystectomy.
