Alkalmazási terület Az ALP működéséhez az optimális ph érték 10 körül van in vitro. Az enzim gyakorlatilag mindenféle szövetben előfordul, különösen nagy mennyiségben található a májban, a csontokban, a placentában, a bélhámsejtekben és a vesetubulusokban. A normál humán szérumban főként a máj és a csont eredetű ALP található. A kettő közötti relatív megoszlást jelentősen befolyásolja a vizsgált személy életkora és neme. A szérum ALP meghatározásnak különleges haszna van a hepatobiliaris betegségek és egyes, fokozott osteoblast aktivitással járó csontbetegségek vizsgálatában. Extrahepatikus elzáródás esetén az enzim aktivitása nagyobb mértékben emelkedik, mint intrahepatikus elzáródásban, és kifejezettebb teljes elzáródás esetén. A csontbetegségek közül a legnagyobb ALP aktivitás növekedést találjuk Paget kórban. Csak mérsékelten emelkedik meg az enzim aktivitása osteomalaciában, és általában normális oteoporosisban. Rachitis esetén 2-4- szeres aktivitás fokozódást látunk, ami csak lassan normalizálódik a D vitamin kezelés kapcsán. Enyhe vagy mérsékelt emelkedést láthatunk Fanconi szindrómában, valamint primer és szekunder hyperparathyroidismus eseteiben. A normál két háromszorosa fordulhat elő a terhesség utolsó harmadában. Egyes rosszindulatú daganatos betegek szérumában a placenta eredetű izoenzim jelenhet meg. A daganatok ugyanakkor képesek más izoenzimek módosult formáit is előállítani. Módszer Kolorimetriás, standardizált kinetikus módszer. Magnézium és cink ionok jelenlétében a p-nitrofenil foszfát hidrolizálódik, alkalikus foszfatáz enzim segítségével p-nitrofenol és foszfát keletkezik. Az ALP teszt eredményét mindig a beteg kórelőzményével, a klinikai képpel és a beteg egyéb laboratóriumi eredményeivel összevetve kell értékelni. (cikksz.: 45301-2-99-80) A teszt korlátai, interferenciák A teszt mérési eredményeit a bilirubin 60 mg/dl (1020 mol/l), a hemoglobin 1000 mg/dl (0.625 mmol/l) valamint a trigliceridek 2000 mg/dl (22,8 mmol/l) koncentrációig nem befolyásolja. Summary ALP has a ph optimum of about 10 in vitro. The enzyme is present in practically all tissues of the body, and it occurs at particularly high levels in liver, bone, placenta, intestinal epithelium and kidney tubules. The predominant forms present in normal human serum can be chartacterized as the liver and bone variants. The respective contributions of these two forms to the total activity are markedly influenced by age and gender. Serum ALP measurements are of particular interest in the investigation of hepatobiliary diseases and bone disease associated with increased osteoblastic activity. The elevation in enzyme activity is higher in extrahepatic obstruction, than in intrahepatic obstruction, and is greater when the obstruction is more complete. Among bone diseases, the highest levels of serum ALP activity are encountered in Paget s disease. Only moderately increased values are observed in osteomalacia and levels are generally normal in osteoporosis. In rickets, levels two to four times normal can be observed, and these fall slowly to normal on treatment with vitamin D. Slight to moderate elevations are associated with Fanconi s syndrome, primary and secondary hyperparathyoidism. An increase of up to two to three times normal may be observed in the third trimester of pregnancy. Placental isoenzymes may appear in the sera of some cancer patients. Tumors may also produce modified forms of nonplacental isoenzymes. Method Colorimetric kinetic assay in accordance with a standardized method. In the presence of magnesium and zinc ions, p- nitrophenyl phosphate is hydrolyzed by alkaline phosphatase to form phosphate and p-nitrophenol. For diagnostic purposes the ALP results should always be assayed in conjunction with the patient s medical history, clinical examinations and other findings. No.: 45301-2-99-80) Limitations and interferences No significant interference up to an approximate conjugated and unconjugated bilirubin concentration 60 mg/dl (1020 mol/l), approximate hemoglobin concentration 1000 mg/dl (0.625 mmol/l) and approximate triglycerides concentration 2000 mg/dl (22.8 mmol/l). There is poor correlation between turbidity and triglycerides concentration. Storage, stability The reagents are stable up to the expiry date on the box and label when stored at 2-8 C. Do not freeze!
