Neuroimaging biomarkers Pharmacological fmri BIOMARKER: a characteristic that is objectively measured and evaluated as an indicator of normal biological processes, pathogenic processes or pharmacologic response to a therapeutic intervention (Lesko & Atkinson, Annu Rev Pharmacol Toxicol 2001) Imaging biomarkers enable the characterization of patient populations and quantification of the extent to which new drugs reach intended targets, alter proposed pathophysiological mechanisms, achieve clinical outcomes as well as predict drug response. Neuroimaging modalities: Neuroreceptor mapping (with PET or SPECT tracers). Structural imaging to examine morphological changes and their consequences. Metabolic mapping (18FDG and magnetic resonance spectroscopy). Functional mapping (FDG PET and fmri) to examine disease-drug interactions. Pharmacological fmri aims at measuring the direct modulation of regional brain activity by drugs that act within the CNS or by the indirect modulation of regional brain activity through pharmacologically modified afferent inputs (Cho et al., 2005; Wise & Tracey, 2006; Paulus & Stein, 2006). For fmri biomarkers to be useful, they need to identify patterns of brain activity that can be correlated with biological events with the aim of validating novel drug targets and predicting drug responses. An fmri biomarker is a fingerprint of changing brain activity that reflects a pharmacological therapy. fmri is of most value at two distinct stages in the process of drug discovery: Neuroscientific investigation of mechanisms of drug action. Providing quantitative markers of drug action, or endpoints, in candidate compounds for the clinic. fmri biomarker for psychiatric disorders fmri biomarkers allow to improve the ability to diagnose psychiatric disorders and develop new treatments. In particular, they provide an opportunity to: Assess the neural correlates of cognitive functions at varying times in either pre-intervention or post-intervention settings. Generate a unique brain processing fingerprint in response to a specific task or stimulus Correlate behavioral pain scores with most important and relevant brain regions fmri biomarkers of psychiatric disorders may offer advantages over the behavioral tests of the patient: They provide an unbiased and less noisy quantification of associated brain activity. Specific biomarkers might exist that represent different constituent components of the cognitive deficits in psychiatric disorders (e.g. attention, working memory, etc.).
