5 ANESZTEZIOLÓGIA, ÉS SÜRGÕSSÉGI ORVOSTAN ORVOSKÉPZÉS folyóirat szerzõi útmutatója A folyóirat célja: Az 1911-óta megjelenõ Orvosképzés legfontosabb célja a hazai orvoskollégák folyamatos graduális és posztgraduális képzésének támogatása. A lap elsõsorban olyan munkák közlését tartja feladatának, amelyek az orvostudomány egy-egy ágának újabb és leszûrt eredményeit foglalják össze magas színvonalon úgy, hogy azok a gyakorló orvoshoz, szakorvoshoz, klinikushoz és elméleti orvoshoz egyaránt szóljanak. Emellett lehetõség van eredeti közlemények és esetismertetések benyújtására, és az újság a Semmelweis Egyetem szakmai kötelezõ szinten tartó tanfolyamok elõadási összefoglalóinak is teret ad. Az eredeti közlemények a rendszeres lapszámokban, vagy a témához kapcsolódó tematikus lapszámokban kapnak helyet. Fontos feladatunknak tartjuk, hogy rezidens kollégák tollából származó esetismertetéseket is közöljünk, melyeket mentori ajánlással kérünk benyújtani. A beadott dolgozatokat a szerkesztõbizottság elõzetes bírálatra adja ki, és a kézirat közlésére a bírálat eredményének függvényében kerül sor. Tudományos dolgozat benyújtására az alábbiak szerint van lehetõség: Esetismertetés (case report) Fiatal doktorok (PhD) tudományos beszámolója, új eredményeinek összefoglalása (nem tézisek vagy doktori értekezések!) Klasszikus összefoglaló közlemény az elméleti és klinikai orvostudomány bármely területérõl, a legújabb irodalmi eredmények felhasználásával Update jellegû közlemény, azaz nem egy téma kidolgozása, hanem adott szakterület legújabb tudományos eredményeinek összefoglalása Elõadási összefoglaló (a tanfolyamszervezõk felkérése alapján) A kézirat: A tudományos közleményeket elektronikusan, Word dokumentum formátumban kérjük eljuttatni a szerkesztõségbe. Az illusztrációkat, ábrákat és táblázatokat külön file-ként kérjük elküldeni. Az ábrák címeit és az ábramagyarázatokat a Word dokumentumban külön oldalon kell feltüntetni, az ábra/táblázat számának egyértelmû megjelölésével. A digitális képeket minimum 300 dpi felbontásban kérjük, elfogadunk tif, eps, illetve cdr kiterjesztésû file-okat. A kézirat elfogadása esetén az ábrákat a szerkesztõség nyomtatott formában is kéri elküldeni. Az orvosi szavak helyesírásában az Akadémia állásfoglalásának megfelelõen, a latinos írásmód következetes alkalmazását tekintjük elfogadottnak. Magyarosan kérjük írni a tudományágak és szakterületek, a technikai eljárások, mûszerek, a kémiai vegyületek neveit. A szerkesztõk fenntartják maguknak a stiláris javítás jogát. A mértékegységeket SI mértékrendszerben kérjük megadni. A kézirat felépítése a következõ: (1) címoldal, (2) magyar összefoglalás, kulcsszavakkal, (3) angol összefoglalás (angol címmel), angol kulcsszavakkal, (sorrendben): magyar cím, angol cím, (4) rövidítések jegyzéke (ha van), (5) szöveg, (6) irodalomjegyzék, (7) ábrajegyzék, (8) táblázatok, (9) ábrák. Az oldalszámozást a címoldaltól kezdve kell megadni és az egyes felsorolt tételeket külön lapon kell kezdeni. (1) A címoldalon sorrendben a következõk szerepeljenek: a kézirat címe, a szerzõk neve, valamint a szerzõk munkahelye, a kapcsolattartó szerzõ pontos elektronikus és postai címének megjelölésével. (2 3) Az összefoglalást magyar és angol nyelven kell beküldeni, külön oldalakon, a következõ szerkezet szerint: Bevezetés ( Introduction ), Célkitûzés ( Aim ), Módszer ( Methods ), Eredmények ( Results ) és Következtetések ( Conclusions ) lényegre törõ megfogalmazása történjék. A magyar és az angol összefoglalások terjedelme külön-külön ne haladja meg a 200 szót (kulcsszavak nélkül). A témához kapcsolódó, maximum 5 kulcsszót az összefoglalók oldalán, azokat követõen kérjük feltüntetni magyar és angol nyelven. (4) A kéziratban elõforduló, nem általánosan elfogadott rövidítésekrõl külön jegyzéket kell készíteni abc-sorrendben. (5) A szövegtörzs szerkezete világos és az olvasó számára átlátható legyen. Eredeti közlemények esetén a Bevezetõ -ben röviden meg kell jelölni a problémafelvetést, és az irodalmi hivatkozásokat a legújabb eredeti közleményekre és összefoglalókra kell szûkíteni. A Módszer részben világosan és pontosan kell leírni azokat a módszereket, amelyek alapján a közölt eredmények születtek. Korábban közölt módszereket esetén csak a metodika alapelveit kell megjelölni, megfelelõ irodalmi hivatkozással. Klinikai vizsgálatoknál a kézirathoz csatolni kell az illetékes etikai bizottság állásfoglalását. Állatkísérletek esetén a Magyar Tudományos Akadémia Egészségügyi Tudományos Tanács állatkísérletekre vonatkozó etikai kódexe érvényes, melyre a metodikai részben utalni kell. A statisztikai módszereket és azok irodalmát is meg kell adni. Az Eredmények és a Megbeszélés részeket világosan kell megszerkeszteni. Referáló közlemények benyújtása esetén a szövegtörzs altémákra osztható, melyeket alcímek vezessenek be. Összefoglaló referátumoknál a szövegtörzs terjedelme ne haladja meg a karaktert (szóközzel), eredeti közleménynél (klinikai, vagy kísérletes) ne haladja meg a karaktert (szóközzel), esetismertetésnél ne haladja meg a karaktert (szóközzel), elõadási összefoglaló esetén pedig ne haladja meg a 8000 karaktert (szóközzel). Irodalom: a hivatkozásokat (maximum 50, elõadási összefoglalónál maximum 10) a szövegben való megjelenés sorrendjében tüntessék fel. A szövegben a hivatkozást a sorszáma jelöli. Hivatkozás cikkre: sorrendben: szerzõk neve (6 szerzõ felett et al./és mtsai), cikk címe, folyóirat neve (Index Medicus szerint rövidítve), év; kötetszám:elsõ-utolsó oldal. Példa: 1. Philadelphia, Lipincott-Raven, 1996; Ábrajegyzék: a megjelenés sorrendjében, arab számmal sorszámozva egymás alatt tartalmazza az ábra címét és alatta rövid és lényegre törõ ábramagyarázatot Táblázatok: külön-külön lapokon kérjük, címmel ellátva és arab számmal sorszámozva. Törekedjenek arra, hogy a táblázat könnyen áttekinthetõ legyen, ne tartalmazzon zavaróan sok adatot. Ábrák: külön-külön lapokon kérjük. Csak reprodukálható minõségû ábrákat, fényképek küldését kérjük (min. 300 dpi felbontásban), a korábban megjelölt file formátumokban. A kézirat elfogadása esetén a nyomtatott ábrát kérjük beküldeni a szerkesztõségbe és az ábra hátoldalán puha ceruzával kérjük jelölni a szerzõ nevét, arab számmal az ábra sorszámát és a vertikális irányát. A formai hiányossággal beküldött kéziratokat nem tudjuk elfogadni. A gyors lektori és korrektúrafordulók érdekében kérjük a legbiztosabb levelezési, illetve címet, telefon- és faxszámot megadni. Elfogadás esetén külön levélben kérjük jelezni, hogy a szerzõk a közleménnyel egyetértenek (és ezt aláírásukkal igazolják), valamint lemondanak a folyóirat javára a kiadási jogról. Írásbeli engedélyt kérünk mellékelni a már közölt adat/ábra felhasználása, felismerhetõ személy ábrázolása, szerzõnek nem minõsülõ személy nevének említése/feltüntetése esetén. A szerkesztõség az általa felkért szakértõk személyét titkossággal kezeli. A kézirat tulajdonjoga a megjelenésig a szerzõt illeti meg, a megjelenés napján tulajdonjoga a kiadóra száll. A megjelent kéziratok megõrzésére szerkesztõségünk nem tud vállalkozni. 7 ORVOSKÉPZÉS Dear Students, dear Audience, It serves for my great pleasure to welcome you all to the Semmelweis International Students Conference. I am delighted to have the possibility to open a scientific meeting uniting 50 young researchers from eleven universities of different parts of the world (Andijan State Medical Institute, Uzbekistan; Asklepios Campus Hamburg, Germany; Bogomolets National Medical University, Ukraine; I. M. Sechenov First Moscow State Medical University, Russia; Medical University of Bialystok, Poland; Peoples Friendship University of Russia; Pirogov Russian National Research Medical University; Poznan Univeristy of Medical Sciences, Poland; Semmelweis University, Budapest, Hungary; University of Latvia, P. Stradins Medical College, Latvia; University of Medicine and Pharmacy of Targu Mures, Romania.). I believe that Budapest is just about to be the perfect place for such an occasion. Following last year s tradition, under the auspices of the Semmelweis International Students Conference, the Semmelweis Students Scientific Association also organizes three practical competitions: a Surgery Practical Competition with individual participation, János Kabay Pharmaceutical Practical Team Competition (in groups of 3-5), and Jolán Bánóczy Practical Competition for Dental Students, individually. The purpose of these competitions is to enable the participants of the conference to compare not only their theoretical scientific qualification, but also their practical, problem-solving abilities with those of the other students from different countries. The conference welcomes papers in the following sessions: basic sciences, clinical sciences, pharmaceutical sciences, and dentistry. Participants also have the great opportunity to attend the lecture of Zhanna D. Kobalava, MD, PhD, DSc, professor - Head of the Department of Internal Diseases, Cardiology and Clinical Pharmacology, People s Friendship University of Russia, on Organ dysfunction in heart failure: overview, current approach and future directions. The motto of this year s conference is the words of a Hungarian poet, Sándor Reményik: Let the smooth paths which take little effort to walk on, and which most of the people long for be the possessions of some others. To me, Almighty, please give rugged pathways leading always upwards, with a beautiful view. These lines teach us that easy and cheap success is not worthy for the vocation of a doctor. If we tried to gain victory without suffers and effort, we would fail in the long run. We should not be frightened by challenges which are there to strengthen us and to make us fit for hard times. We need to sweat and climb the mountain in order to gain real success. Dear students, I would like to wish you a nice and fruitful time here in Budapest, please take the opportunity to discuss many important questions of your scientific field with your young colleagues and build new partnerships. I really hope you will enjoy this conference and will return to your countries with enriching memories that give you new motivation to continue the path you are walking on. 8 ORVOSKÉPZÉS DR. MERKELY BÉLA Tisztelt Elõadók, Résztvevõk és Bizottsági Tagok! Ismét közeledünk az évnek ahhoz a szakaszához, amikor a hallgatók egyetemünk Tudományos Diákköri Konferenciáján mérhetik össze tudásukat és oszthatják meg egymással kutatásuk eredményeit. Az elmúlt év konferenciájának hangulatára nagy örömmel tekinthetünk vissza, amint az Országos Tudományos Diákköri Konferencián nyújtott kiváló eredményekre is. Ezekre alapozva õszintén bízom az idei konferencia sikerességében évi konferenciánkra 344 elõadással jelentkeztek a hallgatók. Az elõzetes bíráló bizottság szakmai véleményezése alapján a Tudományos Diákköri Tanács az elõadások több mint 99%-át fogadta el. Házi konferenciánk felkért elõadója Dr. Kempler Péter, a Semmelweis Egyetem I. Sz. Belgyógyászati Klinikájának egyetemi tanára. Elõadásának címe: Neuropathia diabetica: a kardiovaszkuláris betegségek bûvös kristálygömbje. A tudományos ülés legjelentõsebb támogatója továbbra is a Richter Gedeon Nyrt., amely 2014 óta minden évben elnyeri a Semmelweis Egyetem Tudományos Diákkörének Kiemelt Támogatója címet. Idén is tovább erõsödnek nemzetközi kapcsolataink. A Tudományos Diákköri Tanács kezdeményezésére a konferencián résztvevõ hallgatóknak három gyakorlati versenyen is lehetõségük nyílik a nemzetközi TDK konferencia keretében összemérni tudásukat: a Laparoszkópos Gyakorlati Versenyen egyénileg, a Kabay János Gyógyszerészeti Gyakorlati Versenyen 3-5 fõs csapatokban, illetve a Bánóczy Jolán Gyakorlati Fogorvostanhallgatói Versenyen szintén egyénileg. A versenyek célja, hogy az elméleti megmérettetés mellett a konferencia résztvevõi gyakorlati tudásukat is összemérhessék külföldi kollégáikkal. Az egyetemi konferenciánkon kiváló teljesítményt nyújtó elõadóknak lehetõsége nyílik az Országos Tudományos Diákköri Konferenciára részvételt nyerni ben Pécs ad majd otthont az Orvos- és Egészségtudományi Szekciónak. 12 SEMMELWEIS INTERNATIONAL STUDENTS CONFERENCE, 2017 Practical competition in the framework of the Semmelweis International Students Conference LAPAROSCOPIC COMPETITION The laparoscopic competition will be held during the Student s Scientific Conference The goal of practical competition is to measure basic laparoscopic skills like movements, handling the instruments and suturing techniques. The competition will be organized in two rounds; the final will be among the best 12 students. Qualification: At the first part, students have to place rubber rings from one spike to another spike left to the right and back with time limitation. The first task is to prepare and restore an upper incisor using aesthetic composite material. Participants will receive short theoretical preparation for both topics before the race, then the tasks will also be demonstrated by the lecturers. For the objective evaluation of restorations KaVo Dental teacher system will also be applied. Furthermore, in the current competition for the first time we plan a prosthodontics surprise where the competitors should demonstrate their practical versatility. Date: 9. 17 BASIC SCIENCES Macromolecular Crowding: Effect of high concentration of macromolecules on conformational states of proteins Pfalzgraf Frederik SE Medicine II. Macromolecular crowding occurs in case of high concentrations of macromolecules such as proteins (e.g mg/ml in E. coli) in the cytosol. Since these macromolecules influence the effective concentration of other molecules, they also modulate the function of enzymes and the cell metabolism. The conformational stability of proteins is also influenced by the crowded environment through the excluded volume effect. Since pressure and volume are conjugate thermodynamic variables, use of high pressure in the experiments is advantageous to reveal volume effects. Crowding circumstances are mimicked in vitro mostly by high concentrations of crowding agents like Ficoll and Dextran. Methods: For this study Ficoll (a neutral highly branched hydrophilic polysaccharide with average molar weight of 70 kda) was used as crowding agent, bovine serum albumin (BSA) and lysozyme as test proteins. Ficoll concentration was varied from 0 to 30%, (w/w) while the concentration of the protein was between 5 and 7 % (w/w). Self-crowding was studied using solutions with high test protein concentration (up to 25 %). Fourier Transform Infrared (FTIR) spectroscopy was used to follow the conformation of the test protein. We analyzed the absorption bands of amide vibrations, which are sensitive to the secondary structure. By varying the pressure and temperature, we measured how the crowding condition affects the stability against heat- and pressure denaturation. Results: Form the analysis of the IR-spectroscopic data, it is shown, that concentrations of crowding agents higher than 15% have a stabilizing effect on backbone conformation of proteins. This stability of the secondary structure can be explained by the fact, that an unfolded protein needs higher excluded volume which is counteracted by the presence of the crowding agent. Surprisingly however moderate crowding agent concentrations (below 15%) destabilize BSA against pressure unfolding. Similar effect was found in case of lysozyme. This can be explained by the effect of molecular crowding on the hydration layer at the surface of the protein. This is an important point, because the hydration layer plays an important role in the function of the proteins. Consequently macromolecular crowding is not an insignificant factor for the maintaining of the biological active state of enzymes. Wide known that hypothermia can accure in operation time and in other surgical manipulations if the thermoregulation is not controlled, and that can lead down to other serious health problems. The changes of vital signs, that can accure through unstabile thermoregulation can affect the operation process. That is why scrub nurse need to know the type, lenght of the operation and if there will be used the necessary thermoregulation devices. The base monitoring for patient observation through the operation are electrokardiography, the monitoring of hemodynamic and the monitoring of respiratory system. As an additional monitoring to controle the status of patient through the anestesy is thermometry, also called the measurement of patient body temperature. Termometry is a type of monitoring that has no contraindications. The aim oh the research was to determine the use and meaning of thermoregulation devices intraoperative. In research used methods were the analysis of literature, qualitative research methods, and the research instruments were structured interview andobservation protocol. Of 43 observation protocols, the thermoregulation device is not used in 14 cases, but in 12 cases are used foley blankets, 12 cases an usual blanket and in 5 cases a device with forced air. Operations which are about 4 to 5 hours, and where the thermoregulation device is a usual blanket, in 3 from 12 cases are mentioned, that after operation the patient was shivering. 14 from 43 observations the thermoregulation device is not used, but there is a possibility to use it in operations. All 15 respondents tells, that patient monitoring intraoperative is necessery or very neccessery, because the status of patient is controlled. The feeling for patient, if there is used a thermoregulation device, is better and the patient feels also comfortable, that is the meaning of all interviewed nurses. There are no publications yet. 18 BASIC SCIENCES Tissue testicular inhibin-b as A marker of spermatogenesis Zabrodskiy Yaroslav People s friendship university of Russia Medicine VI. zayardim78@icloud.com Inhibin B is one of the most important serum markers of spermatogenesis, but its testicular expression has been studied to a relatively low extent. Moreover, its sensitivity as well as our ability to forecast the level of its intensity in spermatogenesis of young males are still subject to discussion too. Aims of research: defining the role of Inhibin B in normal spermatogenesis, in case of Sertoli cell-only syndrome and in case of focal spermatogenesis (nonobstructive azoospermia). Methods: we have conducted tests on 82 males diagnosed with infertility, nonobstructive azoospermia. Furthermore, the influence of Inhibin on the germ cells of men aged (n=10) has been analyzed using the Immunohistochemical method. The results are as follows: high expression of Inhibin can be detected both in Sertoli (98.0±2.66%) and Leydig (94.0±1.55%) cells in comparison with the men of the same age in the control group (65.9±0.44% and 12.0±0.44% respectively) when a patient suffers from focal damages of spermatogenesis (mixed atrophy); the level of Inhibin expression in spermatogonia in control group is 4.0±0.22%, whereas in cases of azoospermia it can be characterized by a substantial increase and equals 45.0±0.44%. Conclusions: through analysis of this data has proved that accumulation of Inhibin by Sertoli and Leydig cells takes place, which eliminates it compensatory effect on germ cells according to the principle of inverse relationship. The level of Inhibin expression depends on variants of the Sertoli cell-only syndrome are identified by Sertoli cell morphology, the degree of development of the seminiferous tubules, and the presence or absence of peritubular or interstitial lesions (alterations). ConstantinosVoniatis@gmail.com Hernia is defined as protrusion or expulsion of an organ through the cavity it normally resides. Abdominal hernia is the exit of any part of the gastrointestinal tract through the abdominal cavity due a defect in the abdominal muscular wall. Current first choice treatment involves implantation of a surgical mesh, in order to close the defect and strengthen the surrounding abdominal musculature. Biomaterial science has evolved in recent years yet, currently applied surgical meshes are still accompanied with disadvantages and more common than expected complications including, mesh migration, tissue adhesion, infection and foreign body reactions. A potential replacement of such surgical meshes is poly(vinyl)alcohol. Electrospinning is one of the techniques applied in nanotechnology used for scaffold production and tissue engineering. Furthermore PVA is a biocompatible polymer thus nanofabricated meshes produced by electrospinning, should be biocompatible. Our objective is assessing the mechanical properties of a PVA mesh as a viable implantable scaffold. Mechanical strength is essential as meshes should withstand the intra-abdominal pressure and muscle tension intrinsically created by the body. PVA scaffolds were produced via electrospinning of 9 w% PVA and 1 M GDA solutions. Post electrospinning processing included folding, compression and cross linkage formation via scaffold immersion in 2 M HCl. After washing with ultrapure water, samples were stored in PBS while incubated at 37 OC. Two types of samples(4.5x3.5cm) were prepared, single interrupted and simple running sutured to realistically recreate surgical procedures. Polypropylene mesh samples with the same parameters were prepared serving as control measurements. Assessment of mechanical properties was performed by an Instron 5942.Samples were pulled to a maximal load of 5 N at a speed of 1 mm/s. Different measuring profiles were created examining strength, endurance and elasticity of the samples. Measurements were performed both on air and under liquid. The samples showed gel-rubber like hybrid properties, withstanding the maximum load of 5N with minimal defects only around the suture material. Results reinforced PVA as a viable candidate for new surgical meshes showing its excellent mechanical properties with our current equipment unable to produce force high enough to tear the prepared samples.
